New plans have been unveiled to try and prevent heart disease which in turn could save over 7,000 lives over the next five years. Health professionals in Scotland also hope to prevent 27,000 heart attacks and other “cardiac events” by adopting a new approach.
The plan will provide preventative treatment for around 500,000 people, including risk assessments for all over-40s, in an attempt to stop those most at risk from developing heart disease. It also recommends that more people should be given statin drugs to reduce cholesterol. The plan has been drawn up by Scottish Intercollegiate Guidelines Network (Sign), which draws up guidelines for the NHS. The guidelines also recommend that, for the first time, doctors should take into account social status when deciding whether people need treatment to prevent heart disease.
It is expected that 50% of men in Scotland and 20% of women over the age of 40 could be prescribed cholesterol-lowering statins. As well as being provided with risk assessment and treatment, those with the highest risk of developing heart disease would be given lifestyle advice.
Patients with the most serious type of heart attack should be admitted to regional centres to have the blood clots and narrowed arteries removed and replaced with a device to keep the arteries open. If this is not possible within 90 minutes of diagnosis, they should rapidly receive the most effective clot-busting drugs. For those that are at risk of less severe heart attacks should receive early heart X-rays and be assessed for possible surgery. The guidelines also say that more patients with heart problems should be given implantable defibrillators to reduce the risk of sudden death and discharge arrangements for hospital patients with heart failure should be improved.
Speaking to the BBC, Professor Keith Fox of Sign said: “These guidelines bring together the most robust and up-to-date scientific evidence and the very best clinical expertise to detail how we can save thousands of people from developing and suffering the complications of heart disease.
“The result will be thousands of deaths avoided and tens of thousands of people whose lives will not be blighted by heart attacks, angina, heart failure, heart rhythm disorders and other complications that impair quality of life.”
Last year coronary heart disease (CHD) killed more than 10,000 people in Scotland alone. Deaths have fallen by 30% in the last 10 years due to previous Sign guidelines and it is hoped that these new guidelines will go some way to saving even more lives.
The number of eggs left in a woman’s ovaries are like the grains of sand in an hourglass, ticking away the hours on her biological clock.
Researchers now say they may be able to predict when that clock will wind down.
And while doctors can’t actually count the number of eggs in an ovary, they can measure ovarian volume. British researchers say there’s a direct correlation between the two, and by measuring ovarian volume with transvaginal ultrasound, doctors should be able to predict when menopause will set in and how many fertile years a woman has left.
According to the study authors, this information will revolutionize the care of women looking for assisted reproductive technologies, including those who were treated for childhood cancers as well as women who want to put off starting a family for whatever reason.
Although information still needs to be validated in clinical studies, its benefit is most likely to start with women who are being treated for cancer and women attending fertility clinics, said Tom Kelsey, co-author of the study appearing June 17 in the journal Human Reproduction.
“If women looking for some sort of assisted conception and their physicians know that they’ve got a long time till menopause, then you could plan for a range of treatments,” said Kelsey, who is a senior research fellow at the University of St. Andrews in Scotland. “If you knew menopause was likely in four to five years, you’d plan a different set of IVF [in vitro fertilization] treatments.”
Others reiterate, however, that the findings should be treated with caution.
“Should a young woman who is 30 years old go for a test to figure out whether she’s got three, five or 10 years left on her fertility? Should she make career decisions and life decisions? Are these data good enough to make those determinations?” asked Dr. Alan Copperman, director of reproductive medicine at Mount Sinai Medical Center in New York City. “The answer is obviously no to all of those questions. The predictive value of this test is not good enough to go and tell someone to change their life.”
According to the article, eggs form in a female’s ovary while she is still in the womb, peaking at several million about halfway through gestation and then starting a continuous decline. At birth, there are several hundred thousand and, when menstruation begins, about 300,000. At about age 37, a woman has about 25,000 eggs left, and at menopause only about 1,000.
The time at which menopause sets in is widely believed to be based on the number of eggs reaching a critically low threshold.
The authors of this study measured ovarian volume with transvaginal ultrasound, then looked at the relationship between ovarian volume — ovaries shrink as a woman ages — and number of eggs. They then applied mathematical and computer models to predict menopause.
The study authors are negotiating with a medical school to set up clinical trials. The idea would be to follow women to see if their predictions were indeed correct.
While these authors have come up with a tool to potentially help women plan their lives, a second study in the same issue of Human Reproduction warned that women might not want to leave it too late. Assisted reproductive technology (ART) could not be relied upon to fully compensate for lack of natural fertility after the age of 35, the article stated.
The authors used a computer simulation model to determine that the overall success rate of assisted reproductive technology would be 30 percent for those attempting to get pregnant from age 30, 24 percent for those trying from age 35, and 17 percent from age 40.

The steadily increase in consumer demand for seafood and the concurrent depletion of many wild fish stocks, aquaculture, or “fish-farming,” is now used to produce almost half of the world’s seafood.1 In fact, aquaculture is currently the fastest growing sector of global food production; as wild fisheries continue to be overexploited and mismanaged, aquaculture production is expected to double by 2050.2
Unfortunately, the industrialization of aquaculture has resulted in many of the same environmental and human health problems currently created by livestock factory farms. As in the case of industrial livestock production, many of the harmful impacts of industrial aquaculture stem from the concentration of large numbers of animals within small facilities. In addition to polluting aquatic ecosystems with the enormous volume of waste produced by the fish they confine, aquaculture facilities threaten the environment and human health by releasing hazardous substances such as pesticides, antibiotics, and other drugs into the aquatic environment. Industrial aquaculture operations can also harm natural fish populations by introducing diseases and non-native species into the ocean.
Pollutants Emitted by Aquaculture Facilities
Open water industrial aquaculture facilities typically confine fish within cages or netpens, which allow fish waste, uneaten feed, chemicals, pharmaceuticals, and other pollutants to pass directly into the surrounding water.
Fish Feces. As in the case of livestock manure, when large quantities of fish feces enter an aquatic ecosystem, nutrient levels in the water increase. As described in the Environmental Impacts section, the influx of excess nutrients induces algal growth, and can ultimately lead to eutrophication and the reduction of aquatic biodiversity.3
Uneaten Feed The constant release of uneaten feed causes similar problems; when feed accumulates on the seafloor, it is eventually decomposed by bacteria, which consume oxygen dissolved in the water and can thus create hypoxic “dead zones” below aquaculture facilities.4 The environmental damage caused by fish feces and uneaten feed is particularly severe in enclosed waters with low water exchange rates (ex. lakes, slow-moving rivers, and shallow bays).
Pesticides and Pharmaceuticals. In attempt to prevent disease and epidemics of parasites, industrial aquaculture operations use large quantities of drugs, pesticides, and other chemicals. Although the environmental and human health impacts of many of these substances are not well understood, their use is poorly regulated, reporting requirements are insufficient, and federal oversight is inadequate.5 While some chemicals threaten consumers’ health by leaving harmful residues in fish, drugs and pesticides can also wreak havoc on the environment since many are toxic to aquatic plants and animals.6,7 For more information, read Food & Water Watch’s detailed analysis,
Misuse of Antibiotics When thousands of fish are confined in a small space, it’s easy for disease to spread quickly. Often, industrial aquaculture facilities address this problem by utilizing the same irresponsible antibiotics practices as industrial livestock operations; rather than reducing the density of fish, fish farms continuously administer subtherapeutic doses of antibiotics. This promotes the proliferation of antibiotic-resistant bacteria, which can infect humans.8 The problem is exacerbated by cages and netpens allowing antibiotics and antibiotic-resistant bacteria to pass freely into surrounding waters.
Escapees and Biodiversity Aquaculture facilities also threaten natural ecosystems by facilitating the introduction of nonnative marine species (or domesticated varieties of native species) into the surrounding environment. As a result of net damage from storms, industrial accidents, and attempts by marine mammals to catch farmed fish, many aquaculture facilities release substantial numbers of fish into the environment. For instance, the David Suzuki Foundation estimates that in British Columbia, an average of 90,000 farmed salmon escaped each year between 1990 and 2000.9
Fish that escape from aquaculture facilities pose a significant threat to the viability of existing wild populations – particularly to sensitive populations that have already suffered from overfishing or habitat loss. While nonnative fish species threaten native populations by competing for food and habitat, farmed varieties of native species also pose an environmental threat; after having escaped from pens, farmed fish may breed with their wild counterparts, thereby introducing farmed genetic traits into the gene pool of wild populations. This can dramatically reduce the genetic diversity of the given species in a short period of time.10
Parasites and Disease Despite intensive use of antibiotics and pesticides, crowded conditions within aquaculture facilities foster the proliferation of parasites and disease, which can spread to wild marine species.11 The large number of fish within industrial aquaculture farms provides ample hosts for parasites such as sea lice; epidemics can decimate farmed fish, and also affect wild populations – especially when aquaculture facilities are situated along migratory routes of wild fish.12
Aquaculture facilities can also introduce viral and bacterial diseases into wild fish populations – disease can be spread by live fish that escape from pens, contaminated equipment, fish parts, sea lice, and humans who handle contaminated fish.13 The risk of disease transmission is increased when imported, unprocessed fish are used as feed. For instance, imported pilchards fed to tuna farmed in Australia are thought to have caused viral outbreaks in 1995 and 1998 that killed 75 percent of the wild adult pilchard population in South Australia.14
Depletion of Wild Fish Stocks Some aquaculture facilities use massive quantities of wild fish, which are processed to create feed for carnivorous fish species such as salmon, trout, tuna, cod, and grouper. For instance, experts estimate that 2.7 to 3.5 pounds of wild fish are used to produce one pound of farmed salmon;15 as many as 20 pounds of wild fish are consumed to produce a pound of tuna.16 This resource-intensive form of production depletes the stock of small fish such as anchovy, sardine, pilchard, and mackerel, which can ultimately disrupt aquatic ecosystems by adversely affecting predators such as large fish, marine mammals, and seabirds.17 Researchers are looking for ways to minimize the use of wild fish in feed, but not all the alternatives are positive. Soy is increasingly used to add protein to fish feed; a practice which could encourage the industrial production of mono-cropping genetically engineered soy.
Sustainable Alternatives Fortunately for seafood lovers, certain forms of aquaculture can be conducted without creating ecological disaster. Re-circulating land-based farming operations are able to raise fish in closed systems without emitting pollutants into the surrounding environment; many of these fish farms refrain from using antibiotics, hormones, or chemicals. Small-scale aquaponic operations raise fish and vegetables/herbs in the same system, using the plants to filter out waste and use it as fertilizer. Sustainable aquaculture facilities can also choose to raise omnivorous or herbivorous fish species such as catfish, tilapia, or carp to minimize (or even eliminate) the use of wild fish in feed. In conjunction, sustainable aquaculture and better management of wild fisheries can provide us with long-term access to seafood.
Did You Know? In 2002, more than 630,000 salmon escaped from aquaculture facilities in Norway – that’s more than the total number of Atlantic salmon that spawned naturally in Norway’s rivers!18

15 to 20 percent of all feed can pass into surrounding waters uneaten, thus creating a substantial stream of waste.19
Currently, Asia is the world leader in aquaculture, representing over 90 percent of total tonnage, while North America produces only 1.6 percent.20
More than 80 percent of the seafood consumed in the United States is imported, and the U.S. exports about 70 percent of what is caught or farmed here into countries with higher import standards than are maintained in the U.S.2121

More people are killed at work than die in wars each year.
In the UK about 400 people (employees, self-employed people, and members of the public) are killed in incidents through the operation of commerce each year. From 6 April 2008, companies and organisations can be prosecuted for gross failings in relation to the management of health and safety, where such failings have resulted in fatalities.
The Corporate Manslaughter and Corporate Homicide Act introduces this new offence UK-wide, and is the result of a realisation that the common law principles of manslaughter (which deal with individuals) do not cover the prosecution of companies.
Why has the government introduced this act?
There was a certain amount of inequality, as small businesses were being prosecuted more than larger companies due to a hands-on approach by SME directors. In larger companies, it was harder to prosecute as blame could not be apportioned to one single person. The act removes the obstacle to convicting larger companies.
Previous instances of transport or construction companies being implicated in fatalities include:
· The 1997 Southall rail crash: a high-speed train passed a red signal and collided with a freight train, killing seven and injuring 151 people. The train had a faulty automatic warning system and was being operated with it switched off. In addition, the automatic train protection system was inoperative, and there was no second driver in the cab.
· The capsizing of a P&O Ferry in 1990, resulting in the deaths of 192 people.
· The Paddington crash of 1999, when a train went through a red light and hit another train in west London, killing 31 people. Over 400 people were injured.
All of these instances were the result of failure in Health & Safety management within each company, and could not be attributed to be the fault of any one individual. Now, with the introduction of the new Act, companies can be held accountable. This includes the UK’s 2.3 million companies, partnerships, trade unions, and some non-commercial organisations. In the wake of parliamentary debate, the new law will also apply to deaths in custody, resulting from gross negligence in the prison service or from those controlling police cells.
How will it be applied?
According to the Ministry of Justice, Employers will now legally be obliged to report certain incidents at work, including work-related deaths. The police will lead an investigation if a criminal offence (other than under health and safety law) is suspected. They will work in partnership with the HSE, local authority or other regulatory authority.
The Government expects that cases of corporate manslaughter/homicide following a death at work will be rare as the new offence is intended to cover only the worst instances of failure across an organisation to manage health and safety properly.
Cases of corporate manslaughter will be prosecuted by the Crown Prosecution Service in England Wales and Public Prosecution Service in Northern Ireland. Corporate homicide cases will be prosecuted by the Procurator Fiscal in Scotland.
Health and safety charges may be brought at the same time as a prosecution for the new offence, as well as in cases where it is not prosecuted.
An organisation found guilty of the office will be liable for an unlimited fine, estimated at between 2.5 and 10% of the company’s annual turnover. This means that the current record of £15m could soon be exceeded.
The Act also allows for the imposition of a publicity order, requiring the organisation to publicise details of its conviction and fine. Sentencing guidelines will be available in the autumn of this year.
Public and government functions that are already open to other forms of accountability are to be exempt from the Act, for example, those who apply the spending of public money or military operations. Equally, police forces, child protection agencies and the emergency services are exempt. The new office will, however, apply to the management of custody, although this will be applied at a later date.
Cause for concern?
Companies with secure and efficient Health & Safety Policies should not be overly anxious, however. The new Act will serve only to prosecute those companies that have been shown beyond any reasonable doubt to have recklessly risked the lives of others. An individual cannot be prosecuted under this act, and nobody can be imprisoned. Individuals will still however fall under the Gross Negligence Manslaughter Act.
For legal reasons, the new offence is called “corporate manslaughter” in England and Wales and Northern Ireland and “corporate homicide” in Scotland.
Gwent Healthcare trust has seen the number of medical negligence and personal injury claims made against it rise by a third in 2008, this rise has been described by some in the health care trust as “dramatic”.
The new numbers where released by the organisations medical directorate, revealing some of the increase strains the health authority are under.
The rise of claims in the Gwent healthcare trust mirrors a similar picture being painted across Wales; despite the rise many can’t seem to pinpoint an exact reason as to why there has been a rise apart from the knock-on effect of increased restrictions in the availability of Legal Aid to fight such cases.
The stats speak for themselves really with the number of claims in the year to March 31, there were 110 clinical negligence and 76 personal injury claims registered against the Gwent trust, the total of 186 being 36 per cent up on that for the 12 months to March 31 2008.
The number of claims being made against the trust is the highest they have been since 2001/02, back then the sharp rises in claims followed new reforms in civil law designed to make the process of making a claim cheaper quicker and easier for people to understand.
No win no fee cases on the rise Regarding last year’s increase the Welsh Risk Pool, from which claims against all Welsh NHS trusts are funded, and into which all trusts contribute, states in its annual report that there has been “a rise in the use of conditional fee (no win no fee) agreements to fund clinical negligence cases due to the almost complete withdrawal of Legal Aid, except in cases involving children.
“This means many more High Street practitioners, rather than specialist solicitors, are investigating possible claims against healthcare bodies.”
Gwent healthcare bosses have been looking into the reason for the rises, focusing on the number the of medical negligence claims being made in the accident and emergency where there has been a dramatic rise in the number of claims being made against the department.
The figures show that the number of claims made in 2008/09 is 20 up from 9 made in the previous year, it seems health care bosses are determined to find out if there is a specific problem that needs to be dealt with.
A similar exercise was carried out in the trauma and orthopedics departments in which there were 23 cases of medical negligence in 2008/09 up from the 6 made the previous year.
Despite rigorous investigation into the way the departments run there was no obvious reason for the increase in the number of claims being made against them.
If the investigations into the accident and emergency departments fail to turn up an exact reason for the increase in the number of claim then it may just have to be put down and increase in availability of legal aid, and with a new fast track system being trailed in Scotland it could well be even easier to make a medical negligence claim in the not so distant future.
The nettle plant is native to Europe and can be found throughout the United States and into Canada. This herb was cultivated in Scotland for use in making a durable cloth. The nettle plant is so rich in chlorophyll that it was used by the English to make a green dye for camouflage paint. This camouflage paint was used during World War II.
Nettle is one of the most useful of all plants. It contains alkaloids that neutralize uric acid. By decreasing uric acid, one can help to reduce symptoms of conditions like gout and rheumatism. Additionally, the astringent activity of nettle helps to decrease bleeding. The nettle plant is rich in iron, which is extremely vital to good circulation. It helps to reduce high blood pressure. Tannins that are found in the nettle root have been used as part of an astringent enema. This is used to shrink hemorrhoids and reduce excess menstrual flow. This herb became popular because of its use in irritating the skin of an inflamed area and increasing the flow of blood to reduce inflammation. The stinging action of nettle can be attributed to the histamine reaction that is caused by the formic acid in the hairs. Nettle has a reputation for use in cases of asthma and other respiratory conditions.
The use of nettle root extract was recommended by German physicians for treating urinary retention that is caused by benign prostatic hypertrophy. This recommendation was based upon evidence from clinical studies. Additional studies have determined that nettle root can increase the excretion of chlorides and urea from the urine. The diuretic activity produced by nettle root ahs been confirmed in animal studies. The diuretic properties can be attributed to the high potassium content. However, this has not been verified. A study that was conducted at the National College of Naturopathic Medicine in Portland, Oregon found evidence of nettle for treating hay fever. In this study, freeze-dried capsules of 300 mg were used. The results showed significant relief from hay fever symptoms in the participants.
